It is quite obviously the Czarna Madonna as it shows the streaks on the cheek which are evident on the actual painting and likely happened when it was damaged during the Swedish Deluge. The Czarna Madonna is the symbolic protector of Poland and so is an appropriately symbol of a Polish Monument.
