I don't think the US should be awfully concerned because
1) North Korea doesn't have the missiles to shoot anything to US mainland or outlying islands and
2) they don't have the technology to miniaturize warheads to be mounted on missiles.
Fun fact, in the pacific theater, only China and Russia have missiles, such as the DF series and the RF-29 to reach the US, and these regimes are relatively stable.

The problem here is that the North Korean regime is led by a collection of Nutjobs. Can anybody guarantee that these Nutjobs will not provide a nuclear device to some other entity or terrorist group? Needless to say, the North Koreans are playing with fire. Because the USA will have no problem initiating a first strike if they get wind that the North Koreans are shopping their nuclear weapons.

Well given the fact that North Korea is plagued with famine, a declining economy and an authoritarian government that seems fearful of its own people, if North Korea gets really desperate or if they think they have nothing left to loose, then yeah, they may invade South Korea and use nukes.
